Facility Rental Application

This application is for Facility Rentals only (rental pricing is based on amount of time in facility, you bring your own food/drink, etc.). If you would like to host a Nerf or Gymnastics Party Package where food, drink and staff facilitators are included, please complete the Party Package Application (click here to view).

By submitting this form, you are agreeing to all City of Fairway Facility Rental Terms & Agreements (click here to view) and/or Shawnee Indian Mission Facility Rental Forms & Agreements (click here to view). If you have chosen to add-on a Temporary Alcohol Permit, you are agreeing to all Temporary Alcohol Permit Terms & Agreements (click here to view). 

Once your form has been submitted, a Fairway Parks & Recreation staff member will typically reach out to you within 24-48 hours to confirm your requested rental details (location, date, time, etc.) and payment. Facility rentals are not solidified until payment has been received.
